We are looking for a Registered Nurse to work as a contractor on a Chronic Care Management team. This person will interface with US physicians, physician’s US staff, and Xillium Philippines’ staff in providing chronic care to a group of patients. You may also be providing Remote Patient Monitoring services involving the use of patient home monitoring equipment. You will help in the support and administrative activity to ensure data is properly collected, communicate with patients to discuss the monitoring data and help interpret and explain to the patient. You’ll need to be adept at accessing internet based applications to obtain and review data. You will be given training and support in any systems you need to work with to deliver the remote service. You will be supervised by the physician or care provider and take direction on the goals and processes to follow with the patient as well as collaborate with the care provider both directly and through reporting.
Responsibilities:
Work with physician to implement standards of care within a CCM regimen
Support state level regulatory clinical standards for RPM care
Work with RPM team members to provide full Medicare RPM patient support
Record all activities within the RPM and EMR systems for each patient
Provide documentation and supporting documentation required for Medicare and Insurance reimbursement
Qualities:
Carry a current RN license within the patient’s state or be able to obtain licensing
Able to work independently and virtually from home
Capable of mastering technical tools and applications needed to work remotely
Work on virtual teams with physician and other staff
Qualifications:
Bachelor’s degree in nursing with active state license(or obtainable)
Possess good remote communication skills using chat, email, text, video calling and phone
Demonstrate proficiency at using communication technologies on phone and laptop
